MAIN CAMPUS AND BRANCH CAMPUSES PROPOSAL DATA SHEET
............................................................................................................................................................
The Proposal Data Sheet (PDS) is the University's form that accompanies any University proposal or modification of a proposal, grant, or contract to the Pre-Award offices. The PDS must be completed prior to submittal of the proposal to Pre-Award. The PDS is for internal UNM use only, and should not be included when the proposal is transmitted to the agency. The PDS includes proposal identification data and summarize the features and commitments being made by the University to the sponsor agency. Completion and inclusion of the PDS with a proposal is the responsibility of the PI. For main campus approvals must be obtained from all PIs and from the cognizant chairs, deans, and directors of all PIs and faculty. Signatures on the PDS signifies that the proposal has been reviewed and approved by each signatory, and that the work proposed can be completed as proposed. When there are multiple departments involved, the primary department will prepare the PDS. It is necessary to include the agreed F&A percentage splits with the PDS. These percentages will be used to calculate the return of the F&A costs to each unit by the Vice President for Research or Executive Vice President for Health Sciences. If assistance is required to complete the PDS, work with the department/unit administrator or contact Pre-Award staff. Forms are available at the OVPR web site http://research.unm.edu/forms/unmforms.cfm.
