HSC PROPOSAL DATA SHEET

The Proposal Data Sheet (PDS) for HSC faculty can be found at http://hsc.unm.edu/financialservices/preaward/internalforms.shtml. The PDS for HSC must be approved by the PI and the department chair. The F&A split form is not required with the PDS; for HSC it is submitted at the award stage. The PDS and other internal forms are due to the Pre-Award office five days prior to the due date of the sponsor. In the event that the application is being submitted to NIH and is an electronic submission through Grants.gov, the PDS is due to the PreAward office ten days prior to the due date of the sponsor. If your application is being submitted to NIH and will be a multiple PI project, one Proposal Data Sheet must be signed by all UNMHSC faculty or a separate Proposal Data Sheet may be submitted for each of the individual PIs. If one of the multiple PIs is from Main Campus, the Main Campus PDS must be submitted after approval from the Main Campus Pre-Award office is obtained.

In the event that the Proposal Data Sheet and other internal forms are not submitted according to the deadlines listed above, a timeline waiver is required. The timeline waiver will need to be submitted to the appropriate dean for the PI’s college, i.e., School of Medicine, College of Pharmacy, or College of Nursing. The waiver must include an explanation for not adhering to the appropriate deadlines. The Pre-Award staff cannot proceed until the timeline waiver has been approved. The timeline waiver can also be submitted via email.
